Notary/Apostille Information

A student who graduated from Illinois State University may order a replacement of their diploma or graduate certificate using the Diploma Replacement Form. There is a $12 reprint fee for each diploma or certificate.

If you want notarized copies of your diploma signed by the University Registrar and Notary Public, please indicate so on the Comments: line on the form. There is no fee for this service.

If you want the notarized copies of your diploma to also include an Apostille, you must include the following items with your Diploma Replacement Form.

  • The completed Application for Authentication or Apostille Certifying Documents for Foreign Use stating the country to which the Apostille will be sent
  • $4 check or money order payable to the Illinois Secretary of State ($2.00 for each document needing an Apostille; we provide two notarized copies of the diploma)
  • A pre-addressed (to you or a third party), postage-paid envelope that the Secretary of State will use to mail the notarized diplomas with attached Apostilles

ISU staff will forward these items, along with the two notarized copies of your diploma, to the Illinois Secretary of State Office. They will attach the Apostille and mail the documents using the pre-addressed, postage-paid envelope you provide.

ISU will mail your diploma reprint to you at the address indicated on the Diploma Replacement Form.
